2025-26 Forms
- Petition for Special Circumstances - Income Adjustment - Form is used to petition for extenuating circumstances that your FAFSA does not reflect.
- Petition for Special Circumstances - Cost of Attendance Adjustments - Form is used to request an increase to a student's Cost of Attendance for allowable costs not already included.
- Parent Refusal to Complete the FAFSA - Form is used when a student's parents do not support them and refuse to complete the FAFSA.
Majority of forms will be handled via ProEd.

Other Forms
- Federal Direct PLUS Loan Request Form - Federal Direct Parent PLUS Loans are federal loans for parents of dependent, undergraduate students who are enrolled at least half-time (6 credits) to help pay for their child’s educational expenses.
